1. Duly completed Accident Claim Form
2. Duly completed Attending Physician's Statement Medical report (Part B)
3. Appendix A duly completed by Employer
4. Original Medical Certificate(s) (if Life Assured is self-employed)
5. Certified True Copy of Medical Certificate(s) (if Life Assured is under employment)
6. Original official receipt(s) (for reimbursement benefit only)
7. Original itemized bill(s) (for reimbursement benefit only)
8. Police report
9. Copy of driving license (for claim due to motor vehicle accident)
10. Proof of age for event person, if not admitted (certified IC, passport or birth certificate)
11. Copy of Policyholder's IC (if not submitted before)

1. Duly completed Medical Benefit And Hospital & Surgical Claim Form
2. Duly completed Attending Physician's Statement Medical report (Part B)
3. Original official receipt(s) (including deposit and refund receipt, if any)
4. Original itemised bill(s)
5. Laboratory and Radiology Reports
6. For Overseas claims:
   a. Certified True Copy of passport indicating Biodata, Dates of Departure from Malaysia and Arrival overseas
   b. Original detailed admission bill and receipt (translation of foreign language to English)
7. Original itemized bill(s) (for reimbursement benefit only)
8. Police report
9. Copy of driving license (for claim due to motor vehicle accident)
10. Proof of age for event person, if not admitted (certified IC, passport or birth certificate)
11. Copy of Policyholder's IC (if not submitted before)

2. Duly completed Attending Physician's Statement on Critical Illness Claim
3. Supporting medical report to confirm the diagnosis/illness e.g. biopsy, CT scan, X-Ray
4. Original Policy Contract (where applicable)
5. Proof of age for event person, if not admitted (certified IC, passport or birth certificate)
6. Copy of Policyholder's IC (if not submitted before)

5. Proof of age for an event person, if not admitted (certified IC, passport or birth certificate)
6. Police report, if any
7. Newspaper cutting, if available
8. Medical Board out letter
9. Approval letter and medical report from SOCSO
10. Copy of Policyholder's IC (if not submitted before)

6. Police report for accidental death
7. Certified post mortem report for accidental death

10. Copy of Claimant/Nominee IC (if not submitted before)
11. Copy of Policyholder's IC (if not submitted before)
