- Certified True Copy of Medical Certificate(s) (if Life Assured is under employment)
- Original official receipt(s) (for reimbursement benefit only)
- Original itemized bill(s) (for reimbursement benefit only)
- Police report
- Copy of driving license (for claim due to motor vehicle accident)
- Proof of age for event person, if not admitted (certified IC, passport or birth certificate)
- Copy of Policyholder's IC (if not submitted before)
