By completing a form. Please follow the simple steps;
1. Download and print the Service Request Form (SRF)
2. Fill in the top portion of the form and Part A.
3. The Assured and a witnessed to be sign on page 5 of 5.
4. Once completed you can scan and e-mail to us at customerservice@mcis.my

Mengisi borang. Berikut adalah langkah-langkah mudah :
1. Muat turun dan cetak Borang Permohonan Perkhidmatan (BPP).
2. Isi bahagian atas dan Bahagian A borang tersebut.
3. Pemegang Polisi dan Saksi harus tandatangan pada muka surat 5 dari 5.
4. Apabila telah dilengkapkan anda boleh imbas dan e-melkan kepada customerservice@mcis.my

The CTF is a RM 8 million fund set up by the insurance and takaful industry to support the Ministry of Health’s (MoH) efforts to conduct more COVID-19 testing.
The fund will provide a fixed cash reimbursement up to RM300 for the cost of COVID-19 testing for individuals covered under the individual or group Medical and Health Insurance policies/takaful certificates.
